Living with serious illness often requires much more than medical treatment. People need help understanding their options, someone to talk with about what matters most, support for those caring for them, and connections to services that can ease the burden of a complex system.
Many of those services exist, but too often people don't know about them, can't access them, or find they don't reflect their language, culture, or lived experience.
The Supportive Care Collaborative exists to change that.
Mission
What We Do

We bring together community organizations, city officials, clinicians, health systems, funders, community leaders, patients, caregivers, and other partners across the San Francisco Bay Area to strengthen care and support for people living with serious illness. Through education, partnership, shared learning, and collaboration, we expand access to palliative care, advance care planning, and family caregiver support, with a particular focus on reaching communities that have had the least access.

We envision a future in which all people living with serious illness experience the highest possible quality of life for as long as possible, with care and support that reflect their wishes, values, and needs.
At its heart, this work is about dignity, respect, and connection. Every person, in every community, deserves to face serious illness with the support they need.
