The Supportive Care Collaborative is organized through regional chapters across the San Francisco Bay Area.
Each chapter is shaped by local relationships, leadership, and priorities, while contributing to a broader network of shared learning and action. Together, the chapters form a network that is locally grounded and regionally connected.

Supportive Care Collaborative | San Francisco
Our founding chapter, formerly the San Francisco Palliative Care Work Group, brings together community organizations, clinicians, health systems, and local leaders to strengthen serious illness care across San Francisco.

Supportive Care Collaborative | Peninsula and South Bay
Launched in 2026, the Supportive Care Collaborative Peninsula and South Bay chapter supports San Mateo and Santa Clara counties by bringing together local partners to strengthen care for people with serious illness and those who support them. The chapter has established a Steering Committee member, is hosting monthly community meetings, and is in the process of identifying regional priorities.

Supportive Care Collaborative | East Bay
We are currently launching the East Bay chapter, which will support Alameda and Contra Costa counties. We are recruiting Steering Committee members and engaging community-based organizations and health systems to better understand local priorities.
