Training Trusted Community Educators
Advance care planning works best when it feels personal, familiar, and connected to the people and relationships someone already trusts. PREPARE Together trains trusted staff inside community-based organizations to lead advance care planning workshops for Latino older adults in San Francisco.
The program builds on the evidence-based methodology of Dr. Rebecca Sudore, creator of PREPARE for Your Care, one of the most widely used advance care planning programs in the country. The curriculum is adapted with and for Latino communities to reflect language, culture, values, and lived experience.
How PREPARE Together Works
Each partner organization identifies an ACP Internal Champion, a trusted staff member who is trained through a comprehensive curriculum, coaching, and practice sessions. Partner organizations are compensated for their staff’s time and expertise, including their role in helping adapt the curriculum for the communities they serve.
Champions then lead a three-session workshop series in their own community, with ongoing support from PREPARE Together. Participants learn why planning matters, choose a medical decision maker, talk with that person about what matters most, and begin sharing their wishes with others. The sessions use stories and activities tailored to Latino culture and lived experience.

PREPARE Together is made possible by community-based organizations that already know, serve, and are trusted by their communities. These partners help shape the curriculum and identify staff members to become ACP Internal Champions, who are trained to lead advance care planning workshops in their own community.
Looking Ahead
We are beginning with Latino communities in San Francisco, where the need is clear and strong partnerships are already in place. Over time, we hope to expand PREPARE Together to serve additional communities, adapting the curriculum in partnership with trusted community organizations so it reflects each community’s language, culture, strengths, needs, and lived experience.
PREPARE Together Implementation Guide
By the end of this year, we will release a PREPARE Together toolkit with the full curriculum, evaluation results, and lessons learned from implementation. The toolkit is designed to help organizations bring culturally and linguistically tailored advance care planning workshops to Latino communities and will be freely available to use, share, and adapt.
