About the Program
Advance care planning works best when it feels personal and familiar. When it comes from someone who shares your language, your culture, and your community.

PREPARE Together is built on that insight. Working with trusted staff inside community-based organizations, we train community educators to lead advance care planning workshops designed specifically for Latino older adults in San Francisco.
The program is built on the evidence-based methodology of Dr. Rebecca Sudore, creator of PREPARE for Your Care, one of the most widely used advance care planning programs in the country, adapted with and for Latino communities to reflect their culture, language, values, and lived experience. It is led by Master Trainer Morela Madriz-Ray, whose deep roots in Latino communities and decades of experience in healthcare and education make her uniquely positioned to lead this work.
How It Works
Each partner organization identifies an ACP Internal Champion, a trusted staff member who Morela trains through a comprehensive curriculum and practice sessions.

Champions then lead a three-session workshop series in their own community. Participants explore why planning matters, choose a medical decision maker, talk with that person about what matters most, and begin sharing their wishes with others. Everything is grounded in stories and activities tailored to Latino culture and lived experience.

Looking Ahead
We are beginning with Latino communities in San Francisco, where the need is clear and strong partnerships are already in place. Our long-term goal is broader. We are eager to build on this foundation and expand the model to serve other communities, adapting the curriculum in partnership with trusted community organizations so it is culturally relevant, linguistically appropriate, and responsive to each community’s strengths, needs, and lived experience.

The Toolkit
By the end of this year, we will release a toolkit including the full curriculum, evaluation results, and lessons learned. The toolkit is designed for organizations doing advance care planning work in Latino communities and will be freely available to share and adapt.
